Are you trying to make sense of attention, mood, or school memories from before age 12? The WURS-25 asks adults to rate 25 recollected behaviors and add a total from 0 to 100. This workbook gives you room to answer at your own pace and consider what the number can and cannot tell you. Its two study screening points, 36 and 46, are explained as prompts for further discussion, not severity bands or conclusions.
The 31-page pack comes as PDF files in both A4 and US Letter. Start Here lays out a six-week path and safety notes; Measures contains the questionnaire, scoring ruler, and a worked example. Worksheets help you map memories, reflect on high ratings and school experiences, consider outside evidence, and prepare a conversation. Trackers include score history, a noticing log, memory inbox, and month review. The Quick Reference has a scale summary and three cut-out cards, while the Guide explains interpretation limits and the professional guide supports use with adult clients. Credits and references identify the instrument's authors.
Use one page at a time over six weeks, or follow your own pace. You can repeat marked pages, add context to remembered ratings, and pause if recalling childhood feels heavy; a hard-day worksheet and grounding card are included. At the end, you have a paper record of your recollections, questions, and possible next steps to bring to a qualified professional. The WURS-25 is one source of information in an assessment, not a diagnosis, and a low total does not rule out ADHD.

Questions people ask
How is the WURS-25 scored?
Rate each of the 25 items from 0 to 4, then add the ratings for a total from 0 to 100. The pack explains the study screening points of 36 and 46; they are not severity bands or diagnostic conclusions, and a low score does not rule out ADHD.
Can I print this workbook on A4 or US Letter paper?
Yes. The 31-page workbook is supplied as PDF files in both A4 and US Letter sizes, with the same page counts for each size. You can print only the pages you want to use, and repeat pages marked for copies.
Who should not rely on the WURS-25 score alone?
The WURS-25 is for adults recalling their own childhood, not for rating a child. It is one source of information, not a diagnosis or proof that ADHD is present or absent. A qualified professional considers developmental history, current symptoms, impairment, and other explanations.
