Who it is for

  • Adults reflecting on mood over two weeks
  • People preparing for a care conversation
  • Counselors and clinicians using individual practice handouts
  • People who prefer writing and tracking on paper

Wondering how to make sense of a PHQ-9 result, or how to notice changes between check-ins? The PHQ-9 Mood Workbook gives you space to answer the nine questions for the past two weeks, add the points, and see which of the five score bands includes your total. Its reflection pages help you note what feels difficult, identify what helps or drains you, choose a small step, and prepare to talk with a doctor or mental health professional. A score is a snapshot, not a verdict.

The pack contains 27 pages across eight PDF files, in both A4 and US Letter. Start-Here lays out a six-week path and a safety page. The 11 Worksheets include the measure, scoring, score reflection, a fortnight review, a small-step planner, people-to-talk-to page, appointment prep, hard-day plan, and personal safety plan. Four Trackers provide a score history, re-take sheet, weekly grid, and overview. Two Quick-Reference pages show score bands and cut-out cards; the three-page Guide explains the score and its limits. A two-page Clinician Guide and Credits and References complete the pack.

Use the pages at a pace that suits you: record one two-week period, choose a next step, and repeat the measure every few weeks using the same timeframe. The included path suggests check-ins across six weeks, with dates and totals plotted on the score-history chart. By the end, you will have your own notes about what you reported, what you tried, and what changed over time, ready to review privately or bring to a professional conversation. Print repeatable pages again when you need them.

What it is not

For educational self-reflection, not diagnosis or treatment, and not a substitute for professional care. The PHQ-9 was developed by Drs. Robert L. Spitzer, Janet B. W. Williams, Kurt Kroenke and colleagues, with an educational grant from Pfizer Inc.; no permission is required to reproduce, translate, display, or distribute it. Personal use and individual-practice client handouts are permitted; wider distribution or resale is not covered.

Questions people ask

Is the PHQ-9 free, and what does this workbook include?

The Credits and References page says no permission is required to reproduce, translate, display, or distribute the PHQ-9. This purchase is for the organized 27-page workbook: worksheets, trackers, reference cards, a reading guide, and a clinician guide, supplied as PDFs in A4 and US Letter.

How do I score and read the PHQ-9?

Answer each of the nine questions for the past two weeks using 0 to 3 points, then add them for a total from 0 to 27. The Guide and Quick-Reference show five bands: minimal, mild, moderate, moderately severe, and severe. The score is a screening snapshot, not a diagnosis.

Can I print the PHQ-9 workbook on A4 or US Letter paper?

Yes. The pack includes PDFs in both A4 and US Letter, and the included report confirms all eight files have the same page counts in each size. The complete workbook is 27 pages in either size. Repeatable worksheet and tracker pages can be printed again for personal use.
