Wondering what your PCL-5 score says about the reactions that bothered you during the past month? The 20-item measure can help you record those reactions, but a total alone cannot explain why you feel this way or establish a diagnosis. This workbook gives you a calm paper route from answering the items to deciding what questions or reflections feel useful.
The Measures PDF contains the questionnaire and a hand-scoring page for the total and four clusters. Start-Here lays out a first session and a six-week path. Thirteen worksheets include pages on reminders, avoidance, thoughts, sleep, and a hard day; five trackers include weekly check-ins and a score history chart. The Quick Reference has score facts, appointment questions, and a repeatable support plan. The Reading Guide explains the provisional 31–33 screening range and the limits of a score, while the Professional Guide and Credits explain administration, authorship, references, and licence. Each file is a PDF supplied in A4 and US Letter; the complete set is 33 pages in either size.
Begin with the questionnaire and scoring page, then choose one worksheet that fits what you want to notice. You can follow the suggested six-week sequence, pause or skip a step, and copy the pages marked Repeat. If you repeat the measure, record the month in the re-test tracker and compare entries as a prompt for reflection or a conversation with a qualified professional. You finish with written observations, questions, and a support plan you can keep nearby.

What it is not
For educational self-reflection only; it is not a diagnosis or treatment. The PCL-5 was developed by Frank W. Weathers and colleagues with the U.S. National Center for PTSD. Personal buyer use and professional client reuse are included; wider group and institutional licences are separate.
Questions people ask
Is the PCL-5 free, and what does this workbook include?
The workbook credits the PCL-5 source page and reproduces its items, response options, and scoring. This purchase is for the organized printable workbook: questionnaire and scoring, worksheets, trackers, quick-reference cards, guides, and credits. Personal use and professional reuse with clients are included; wider group and institutional licences are separate.
How do I score and read the PCL-5 workbook?
Rate each of 20 items from 0 to 4 for the past month, add them for a 0–80 total, and sum the items in four clusters. The guide describes 31–33 as a provisional screening cut-off range in some samples. A score is not a diagnosis or a severity level.
Can I print the PCL-5 workbook on A4 or US Letter paper?
Yes. The workbook files are provided as PDFs in both A4 and US Letter. Each version has 33 pages across the cover, questionnaire and scoring, Start-Here, worksheets, trackers, quick reference, reading and professional guides, and credits.
