Who it is for

  • Adults reflecting on close family and friend ties
  • Older adults using a structured paper exercise
  • Clients working through pages with a professional
  • Counsellors and social workers supporting reflection

If you are wondering how many close family members or friends you see monthly, could call on, or can talk with about private matters, the LSNS-6 offers six questions to organize that reflection. The scoring page helps you add a family score, a friend score, and a total from 0 to 30. A fictional worked example shows how the arithmetic fits together, while the guide explains suggested screening flags as limited prompts rather than conclusions about an individual.

The 32-page workbook is supplied as PDFs in A4 and US Letter. Start Here lays out a six-week path; Measure and Scoring includes the questions, scoring steps, and example. Twelve worksheets include a network map, contact log, reach-out cards, a three-rung ladder, and prompts for difficult moments. Five tracker pages offer score history, contact grids, and weekly and monthly reviews. A scoring card and a cut-out My Three People card sit alongside a three-page reading guide, a professional guide, and credits and references. The separate PDFs work together from first answers through reflection and tracking.

Use a pen to complete the questions, then choose one worksheet at a time over the coming weeks. Repeat the contact pages as needed, revisit the six questions when it suits your own plan, and note scores and contact patterns in the trackers. You finish with a written snapshot of your answers, named ties and possible supports, plus a record you can review or bring to a conversation with someone you trust or a qualified professional.

What you get

  • Measure and Scoring walks through six questions, family and friend subtotals, and a total score from 0 to 30.
  • A fictional worked example shows how the scoring page is completed without presenting its result as a prediction.
  • Network map, reach-out cards, and the three-rung ladder help turn counts into names and manageable contact plans.
  • Trackers include score history, contact grids, weekly reviews, and a monthly overview for recording patterns over time.
  • Quick Reference pairs a scoring card with a cut-out My Three People card for keeping chosen contacts close.

What it is not

For educational self-reflection only; it is not a diagnosis or treatment. The Lubben Social Network Scale and its six-item form were developed by James Lubben and colleagues. Personal-use licence; professionals who purchased it may print and reuse client-facing pages with their own clients.

Questions people ask

What does the LSNS-6 score mean?

The six answers are scored from 0 to 5 each. Add items 1–3 for a family score out of 15 and items 4–6 for a friend score out of 15; together they make a 0–30 total. The guide describes below 12 total and below 6 on a subscale as suggested flags used in some older-adult research, not proof or diagnosis.

Can I print the LSNS-6 workbook on A4 or US Letter?

Yes. The workbook files are PDFs provided in both A4 and US Letter, with matching page counts. The pack contains nine PDFs and 32 designed pages in total. The trackers and other pages marked Repeat can be printed again when you want additional copies.

How often should I repeat the LSNS-6?

The workbook does not prescribe a repeat interval. Its six-week path includes answering the questions again in Week 6 and recording both scores on the score history chart. You can use the repeat trackers to keep your own notes without treating a change in score as a diagnosis or guaranteed outcome.
